Joe McCarthy's injury is a major source of concern as the British and Irish Lions finalise preparations for a series win over Australia on Saturday.
McCarthy limped during the 27-19 win over the Wallabies in the first Test match with plantar fasciitis and has been unable to train since, putting his participation in the replay at the Melbourne Cricket Ground in doubt.
The Irishman's final opportunity to show his fitness before Andy Farrell announces his team will be during training on Thursday and it is likely he will miss it.
James Ryan and Jack Morgan were pulled out early in the second half of Tuesday's 24-19 win over a First Nation and Pacific XV, which could require changes to the squad ahead of the second Test against Australia.
If McCarthy is unable to make the field, Ollie Chessum could move into the second-row position alongside captain Maro Itoje, creating a vacancy on the bench, while another option would be to move Tadhg Byrne from wide to centre.
If Farrell decides to move Byrne to another position, Morgan will become a candidate for the 23rd round.
Given the Lions' dominant performance against the Wallabies at Suncorp Stadium, any surprise changes to the line-up are unlikely.
